To have a family doctor in the PrivaMED network, you must become a member by signing in to a subscription. You only pay for one annual subscription to gain access to the entire network. The subscription gives each member of the subscription personal access to Internet services. Access to the electronic health record is individual for people 14 years of age and over.

There are three types of subscription**:

• Solo: one person___________ annual rate of $ 145
• Duo: two people_______________ annual rate of $ 245
• Family*: three to six people___ annual rate of $ 295

*Based on a maximum of two people aged 21 and over and four people under age of 21. GST and QST are added to the subscription prices.

** Payment for a subscription can be made on an annual or monthly basis. Subscriptions are always for a period of one year, renewable by tacit agreement.

The price of consultations is determined according to the type of consultation and includes, if necessary, the preliminary appointment with the nurse.

• Medical follow-up :  $ 110-130
• Health check-up / Support:  $ 195-240 
• Telephone follow-up  * :  $ 75-130 (Telephone consultation must be prepaid)

* Note that medical and laboratory costs are not included *

• Forms for government and other organizations (SAAQ, CNESST, RRQ, Hydro-Québec, GRC, etc.) : from $ 75
• Various certificates (absence, ability, presence ...) : from $ 25
• Costs of medico-administrative activities (File Management / Revision) : from $ 75
• Results interpretation fees : $ 8

For some needs, a nurse consultation may be more suited to your needs. If this is the case, we also have consultations for which the basic rates are established according to the time you spend with the nurse.

• Assessments (ADHD, Alzheimer's disease screening, Psychological) : $ 95

Depending on the services required and / or a particular context, incidental / additional costs may be added to your billing:

Administrative cost :

• Surcharge “outside working hours” (Saturday morning/Holiday) : $ 45

Contextual fees :

• Additional time :$ 8 / min.
• Cancellation fees * : equivalent to the fees for the planned consultation

* You can cancel your appointment at any time using Internet or during business hours by phone, as long as you respect the cancellation date and time that are mentioned to you when you make the appointment. If you cancel an appointment after the cancellation limit, your member file will include cancellation fees at the current rate and it will no longer be possible, until these fees have been paid, to make an appointment in all the clinics of the network, and that for all the members of the subscription. If you arrive more than 5 minutes late for your appointment and the clinic cannot find an alternative, your appointment will be considered to have been canceled after the cancellation deadline.
